National papilloma database, HPV related disease in ORL in NZ

I have finally gotten the National papilloma database set up and ready for everyone to enter data. This is for any HPV related disease in ORL in NZ. I need anyone who wants to use it to email me with their Name, DHB, email address (institutional probably best) so that I can get particular log in's made for each person.
